Our team works alongside participants, families, and other professionals to ensure that supports are meaningful, coordinated, and responsive to changing needs.

Support Coordination

Support Coordination helps participants understand and implement their NDIS plans. Our role is to assist individuals in connecting with the right services and supports so they can work toward their goals.

We support participants by:

  • Understanding their NDIS plan and available funding
  • Connecting with appropriate service providers
  • Coordinating services and supports
  • Helping build confidence and independence in managing supports
  • Navigating the NDIS system and resolving challenges when they arise

Our goal is to ensure participants feel confident and supported in making decisions about their care and future.

Specialist Support Coordination

Specialist Support Coordination is designed for individuals with more complex needs who require higher levels of coordination and support.

Our team assists participants who may be experiencing:

  • Complex psychosocial or behavioural challenges
  • Housing instability or risk of homelessness
  • Justice system involvement
  • Crisis situations requiring coordinated support
  • Multiple service providers and complex care arrangements

We work closely with participants, families, allied health professionals, and community services to develop coordinated strategies that promote stability, safety, and long-term wellbeing.

Psychosocial Recovery Coaching

Psychosocial Recovery Coaching supports individuals living with psychosocial disability to build resilience, develop life skills, and strengthen their independence.

Our recovery-focused approach helps individuals to:

  • Build confidence and self-determination
  • Develop strategies to manage daily life
  • Strengthen social connections and community participation
  • Identify personal goals and work toward meaningful recovery outcomes

We recognise that recovery looks different for everyone, and we walk alongside individuals as they move forward in their own way and at their own pace.

Capacity Building

Capacity Building supports focus on strengthening the skills and confidence individuals need to live more independently and participate fully in their communities.

This may include support with:

  • Developing daily living skills
  • Building confidence and decision-making abilities
  • Strengthening social connections
  • Accessing education, employment, or community opportunities
  • Navigating complex systems and services

Our aim is to empower individuals with the tools and confidence they need to live meaningful and fulfilling lives.

Community Participation

We support individuals to connect with their communities, build relationships, and participate in activities that are meaningful to them.

This may include:

  • Engaging in social or cultural activities
  • Accessing community groups or programs
  • Building friendships and social confidence
  • Participating in hobbies or interests

Community participation plays an important role in supporting wellbeing, independence, and a sense of belonging.


